For New Clients of A R BYRNE & ASSOCIATES, all mandatory fields must be completed. 120 Points of Identification must be submitted with your completed form, which must contain at least one (1) form of Photo Identification, such as an Australian State or Territory Drivers Licence, NSW Photo Identification Card or Australian Passport. All information is provided for our records, and remains confidential at all times.
CREDIT CARD AUTHORITY FORM (.PDF)
For Clients of A R BYRNE & ASSOCIATES that are unable to attend our Office in-person, all mandatory fields must be completed. VISA, MASTERCARD and AMERICAN EXPRESS (Debit and/or Credit Cards) are accepted by completing this form. A flat 1.4% surcharge applies to all types of Credit Cards. All completed forms are to be returned to our Office by 3pm Monday to Friday for same-day processing, or thereafter for next-business day processing.
